Emergency pharmacology questions tend to cluster around a relatively small set of high-consequence medications. Understanding why a drug works — not just its dose — makes these questions far easier to reason through, even when the exact scenario is unfamiliar.
Reversal agents and antidotes
Know your pairs: naloxone for opioids, flumazenil for benzodiazepines (used cautiously), N-acetylcysteine for acetaminophen, sodium bicarbonate for tricyclic antidepressant cardiotoxicity, atropine and pralidoxime for organophosphates, and 4-factor prothrombin complex concentrate for warfarin-associated major bleeding.
Vasopressors
Norepinephrine is first-line in septic shock. Understanding why — its balance of alpha and beta effects with a more favorable arrhythmia profile than dopamine — helps you reason through related questions rather than memorizing a rule in isolation.
Resuscitation medications
Know first-line agents for status epilepticus (benzodiazepines, adequately dosed), anaphylaxis (intramuscular epinephrine), and unstable bradycardia or tachycardia per ACLS algorithms.
Common traps
Exam writers often test the almost-right answer: a real medication used for the wrong indication (e.g., protamine for warfarin instead of heparin), or the right medication given by the wrong route or with an unsafe delay. Read every answer option carefully, including the ones you're confident are wrong — understanding why they're wrong is what builds durable knowledge.
